Ready to start your journey towards rewards? The first step is to fill out the form below sharing information about yourself and your referral. Once you have completed the form, if your referral is hired and starts employment you can submit a payment request within the first 90 days of their employment in PeopleSoft using the referral award form.
The following roles are eligible for a referral bonus:
- Biomed Tech.
- Clinic Office Receptionist and Specialist.
- CMA.
- CNA 1 & 2, ER Tech, OB Tech, OR Tech.
- Dietitian.
- Echocardiographer.
- Housekeeper.
- Human Resources - Recruiter/Sourcer, Representative, Business Partner, Analyst/Specialist.
- Imaging - Angio, Cardiac Cath Lab, CT, Mammography, MRI, Nuc Med, Ultrasound, Vascular.
- IS - Administrator, Architect, Engineer, Developer, Epic Analyst, Informaticist, Technical Project Manager, Trainer, Web Design.
- Lab - Cytotechnologist, Histologist, Lab Assistant, MLS, MLT, Phlebotomist.
- LPN.
- Nutrition Services.
- Perfusionist.
- Pharmacist & Pharmacy Technician.
- Radiation Therapist, Medical Dosimetrist, Medical Physicist.
- Respiratory Therapist.
- RN - Ambulatory Surgery, Dialysis, ICU-CCU, Emergency, Labor & Delivery, Med Surg, Mental Health, Oncology, PACU, PCU, Surgery Main OR.
- Sleep Study Tech.
- Social Worker.
- Therapist - Hand Therapist, Physical Therapist, Occupational Therapist, Speech Therapist.

The award amount (according to policy), which is subject to federal, state and local taxes, for each job title will be:

| CNA 1 (minimum one-year experience) or CNA 2 (must already have CNA 2) with less than six months experience | $500 |
| CNA 2 greater than six months experience | $1,500 |
| Housekeeper Dietary/Culinary Aide Cook | $1,000 (0.5 FTE or greater) |
| Physician | $2,500 |
| All others indicated on list | $1,500 |
*Up to $2,500 is available for Physician referrals.
